We are not permitted to share any medical details with a third party without your consent. If you wish to give a family member, friend or any nominated person consent to your medical data please complete a form.
You can either give the nominated person full consent or just a specific area of consent. You can also give this consent on a permanent basis or for a period of time. Once you have filled in the form you can either bring the form into reception or email the completed form to: [email protected]
Please be aware that it is the patient’s responsibility to inform us if they change their mind and wish to remove any consent to share their medical information with the nominated person they have given consent to.
