We may ask for you consent to share your data with other services such as the hospital, physio or District Nurse. For example if you needed to be referred to a specialist we can only do this with your consent to share.
Examples of other data sharing services can be commissioning, planning, rick stratification, patient identification, which is were your information is used for planning in the area due to the population. They use the data to work out how many GPs etc we need for the growing population.
For new patient's registering at the surgery you will be asked to fill in a consent to share form.
