If you are eligible you will either receive a text message from the surgery, a letter from NHS England or a phone call from the surgery to offer an appointment. If you are unsure if you are eligible please click the link below to visit the NHS website or contact the surgery directly.
You can book your appointment either via the text message you received, calling the surgery or visiting the surgery and speaking to a staff member on reception.
The appointment will be at The Baffins surgery.
