As a surgery, we are changing the way we review our patients with any long term conditions.
What are the changes?
- You will now be invited depending on your birth month - for example if you were born on 19th June you will be contacted by us in May to book your long term condition review in June.
How will this effect me?
- It may mean that you are seen sooner than you think you may need to. For example you may of only been seen in February but because your birth month is May you will be contacted in April.
How will I be contacted?
- We will contact you via SMS or phone call - in some cases where we have no numbers recorded we will send a letter to you in the post.
What happens if I am housebound?
- Do not worry as we will still plan reviews with but the time frame may differ.
